What can I drink besides water that won’t spike my blood sugar?

Besides water, you can enjoy unsweetened coffee, tea, and low-calorie drinks without spiking your blood sugar. Research also shows that specialized beverages—such as whey protein pre-loads, fresh pomegranate, and bitter melon juices—can actively help your body process sugar more effectively.

1. Green tea

Green tea consumption was shown to lower fasting glucose by an average of 0.09 mmol/L (1.62 mg/dL). It significantly reduced HbA1c by 0.30%, suggesting that regular consumption provides a durable glucose-lowering effect over time.

Green tea catechins, especially EGCG, help manage blood sugar through four key mechanisms:

  • Boosts cell uptake: Increases GLUT4 transporters to help muscle and fat cells absorb glucose.
  • Protects insulin production: Preserves pancreatic islet cells.
  • Blocks sugar digestion: Inhibits carbohydrate-digesting enzymes (alpha-amylase and alpha-glucosidase).
  • Reduces liver output: Suppresses glucose production in the liver.

2. Pomegranate juice

Fresh pomegranate juice shows strong potential for managing blood sugar. At a dose of 1.5 mL per kg of body weight, fresh juice demonstrated clear glycemic improvements in patients with type 2 diabetes.

Pomegranate juice contains punicalagin, which directly stimulates insulin release. Punicic acid activates PPAR receptors to improve glucose tolerance and reduce inflammation. Polyphenols & antioxidants protect cells from oxidative stress, a driver of insulin resistance.

Large-scale long-term studies show that high intake of fruit juices can raise overall type 2 diabetes risk due to their natural sugar content. To get the benefits, stick to fresh, 100% pomegranate juice in small, controlled portions—not large volumes of processed or sweetened brands.

3. Strawberry and Cranberry Blend

A clinical trial involving insulin-resistant individuals found that consuming a beverage with 333 mg of polyphenols from strawberries and cranberries daily for six weeks significantly improved insulin sensitivity.

4. Blackcurrant and Apple Combinations

Drinks rich in blackcurrant anthocyanins (often studied alongside apple polyphenols) have been shown to lower post-meal plasma glucose, insulin, and C-peptide levels in healthy adults within just 30 minutes of consumption.

5. Blueberry Juice

In vitro studies show that blueberry anthocyanins (specifically malvidin-type) significantly boost glucose consumption in cell lines. One case report noted that a pre-diabetic patient experienced a profound lowering of serum glucose (from 105 mg/dL to 30 mg/dL) after regular consumption of blueberry juice.

6. Açaí Beverages

One study on metabolic syndrome patients found that açaí drinks (rich in anthocyanins) did not significantly change glucose levels. They did significantly decrease markers for inflammation and oxidative stress, which are known drivers of diabetes complications.

7. Sea Buckthorn

Smoothies made from these berries have demonstrated the ability to inhibit pancreatic lipase, helping to reduce energy intake and support overall glycemic control.

8. Bitter melon (bitter gourd) juice

A study found that daily consumption of 45 mL of a fermented bitter gourd beverage for six months led to a significant reduction in fasting blood glucose, postprandial (post-meal) blood glucose, and HbA1c levels

It contains charantin and polypeptide-P, compounds that have blood glucose-lowering properties very similar to insulin. Research shows the juice stimulates the uptake of glucose into muscle and fat cells, particularly by increasing the tissue’s sensitivity to insulin

While bitter melon juice is powerful on its own, research also shows it can be used to fortify other beverages. For example, tigernut beverages fortified with bitter gourd extracts showed a significant increase in their ability to inhibit sugar-breaking enzymes

9. Coffee

Large-scale cohort studies consistently link the consumption of coffee and tea to a 2–10% lower risk of type 2 diabetes when used to replace one daily serving of a sugary beverage.

One study suggests that replacing sugar-sweetened beverages (SSBs) with coffee or tea can result in a 20% decrease in the development of diabetes.

Clinical guidelines and research summaries generally recommend unsweetened coffee as “simple and low-cost options to improve glycemic and weight control”

10. Low- and no-calorie sweetened beverages (LNCSBs)

LNCSBs do not contain real sugar, so they do not cause the immediate postprandial rise (the high-sugar rush after drinking) that standard sodas or juices do.

In fact, large-scale population data (NHANES) has shown that people who drink LNCSBs instead of water or sugary drinks often have lower hemoglobin A1c (HbA1c) and lower insulin resistance markers (HOMA-IR).

Clinical research confirms that replacing sugar-sweetened beverages (SSBs) with diet versions leads to significant reductions in body weight, BMI, and even liver fat.

However, long-term observational data show a statistical association between LNCSB consumption and increased metabolic disease risk. This is largely explained by reverse causation. People already at high metabolic risk disproportionately switch to diet drinks, skewing the data.

The short-term glucose neutrality of LNCSBs is well established; the long-term picture is less clear. They are a useful transitional tool for people moving away from sugary drinks, not a long-term substitute for water or tea.

11. Milk

Replacing one daily serving of a sugar-sweetened beverage (SSB) with reduced-fat milk (0–2% fat) is associated with a 2–10% lower risk of type 2 diabetes.

However, it is important to distinguish between plain and sweetened milk. Consuming milk with added sweeteners is associated with a higher risk of diabetes development. This places it in the same category as other sugar-sweetened beverages

12. Whey protein pre-load

A 15g dose of intact whey protein taken immediately before breakfast reduced the total post-meal blood sugar rise by 13%. In clinical tests, the improvement in blood sugar levels was sustained through both breakfast and lunch meals when the 15g dose was repeated.

Consuming whey before meals was shown to significantly reduce “peak” hyperglycemia (the highest point your blood sugar reaches after eating)
